At Recovery House of Central Florida in Sanford, Florida, every client who is enrolled in the Life Rebuilding Program receives comprehensive case management services. These case management services are designed to foster accountability, growth, and independence.
Individualized Service Plans are developed and completed by client and their case manager within the first 30 days of enrollment. These personalized plans are tailored to address key life domains, which include: employment; education and job training; social relationships; mental health; legal issues; family reunification; and, physical health.
Clients receive ongoing support by meeting with their case manager every 30 days. During this time, a client’s progress is reviewed and goals are adjusted, as needed. Ongoing support includes an emphasis on goal-setting, resource navigation, self-advocacy, and developing skills for life beyond recovery.
The ultimate goal for Case Management is to equip clients with the ability to set realistic goals, seek out community resources, maintain healthy relationships, and continue their personal growth journeys after graduation. This structured, trauma-informed approach ensures that each client has a clear path forward, along with the support to walk it confidently.
